Coordinating care across Colorado and Nebraska

Overview

Sedgwick County Health Center partners closely with providers and facilities across Colorado and Nebraska to make sure every patient receives the right care, in the right place, at the right time. Whether you’re referring a patient to SCHC for hospital or specialty services – or you’ve been referred from SCHC to another care team – this page explains what to expect.

Referring to SCHC

For community providers and partner clinics

We welcome referrals for:

  • Inpatient and Hospital to Home (Transitional Swingbed) Care
  • Specialty Clinics (Cardiology, pain management, etc.)
  • Imaging and Lab Services
  • Rehabilitation and Therapy Services
  • Long-Term Care and Assisted Living placement

What happens next:

Our referral coordination team will review your submission within 24–48 hours and contact the patient directly to schedule their visit. We’ll provide status updates and return clinical notes to you after the visit for continuity of care.

Referring from SCHC

For patients and families

Sometimes your SCHC provider may recommend follow-up care with a specialist or facility outside our organization — for example, advanced imaging, surgery, or subspecialty consultation.

Here’s what you can expect:

  • Our care team will send your referral directly to the receiving provider or facility.
  • You’ll be contacted to schedule your appointment.
  • We help coordinate insurance authorizations and share your records securely.
  • If you haven’t heard about your referral within a few business days, call 970-474-3323 or message us through the Patient Portal.
