During Your Visit

We’re here to make your visit easy, comfortable, and personalized.

This guide is for clinic, therapy, & specialty clinic appointments. Visiting for a hospital stay, lab, or imaging? See those specific pages for details.

Bringing a Family Member or Friend

You’re welcome to bring a family member, friend, or caregiver to your appointment for support.
They’re encouraged to join you in the exam room if you’d like — we believe care is stronger when shared with those who matter most.

Checking In

When you arrive, come to the front desk of the location you’re visiting. Our team will greet you, confirm your information, and help you get checked in for your appointment.

Please have your:

  • Photo ID
  • Insurance card
  • Copay or payment (if one is due)

Tip: Try to arrive about 15 minutes early to allow time for check-in and signing forms.

During check-in, you’ll be asked to review and sign a couple of short forms on our secure iPads:

  • Consent for Treatment
  • Notice of Privacy Practices

These forms make sure you understand your care and privacy rights.
Our staff can answer any questions before you sign — we’re happy to help!


Each of our locations has a comfortable waiting area where you can relax while we prepare for your visit.

Meeting Your Care Team

Once you’re checked in, a nurse or medical assistant will:

  • Take your vital signs (like blood pressure, temperature, height, and weight)
  • Ask about your current medications and allergies
  • Review the reason for your visit

Then, you’ll meet your provider to talk about your health, answer questions, and make a plan together.

We encourage you to bring a list of questions or symptoms you want to discuss – we’re here to listen.

Communication & Comfort

We know doctor visits can feel stressful, so our team focuses on clear communication and care that feels personal.

Please let us know if you:

  • Need extra time to ask questions
  • Prefer your provider to explain things in a certain way
  • Need an interpreter or accessibility support

We’ll slow down, listen carefully, and make sure you feel comfortable and understood.

Rehabilitative Therapies Visits

When you arrive for your rehabilitative therapies appointment, please check in directly at the Therapies office, located just across from Main Reception at Sedgwick County Health Center in Julesburg, CO.

  • The check-in process is the same, and our team will help you get settled.
  • Bring any referral forms or records your primary provider sent with you.

Lab Work, Imaging & Referrals

Sometimes your provider may order:

  • Lab tests or X-rays
  • Referrals to specialists
  • Follow-up appointments

Our team will guide you through every step – scheduling, instructions, and what to expect next.

If you have tests done, you’ll get results through the Patient Portal or a phone call from our clinic team.

Specialty Clinic Visits

If you’re seeing a visiting specialist (like Cardiology or Pain Management):

  • These providers visit on select days each month – your clinic staff can confirm where to check in.
  • The check-in process is the same, and our team will help you get settled.
  • Bring any referral forms or records your primary provider sent with you.

After Your Appointment

Before you leave, your care team will:

  • Review your visit summary
  • Schedule follow-up appointments if needed
  • Make sure all your questions are answered

You can always log into the Patient Portal to view visit notes, test results, and upcoming appointments.
