Appointment Preparation

Bring these items with you:

A photo ID (driver’s license, state ID, or passport)
Your insurance card(s) and any copay or payment due at check-in
A list of your current medications (including prescriptions, over-the-counter, vitamins, or supplements)
Any referral forms, imaging discs, or lab results your provider requested
Your completed pre-visit forms (complete in patient portal)

Tip: Arrive 15 minutes early to allow time for registration and check-in.
