How to Get Started

Option 1: Self-Enroll Online

If you are an existing patient, you may be able to create your portal account online.

You must:

  • Be a current patient in our system
  • Be age 13 or older
  • Be signing up for your own account (not a caregiver or proxy)
  • Have your date of birth AND either: your email address or social security number on file.

Note: Self-enrollment will only work if your information matches what we have in our system. If you’re unsure what email we have on file or run into issues, please call us and we’ll help you get set up.

Option 2: Get an Invitation by Phone or Visit Us in Person

If self-enrollment doesn’t work or if you prefer help:

  • Call reception at 970-474-3323 or visit us in person
  • Tell us you’d like Patient Portal access
  • We’ll send you a secure email invitation
  • Follow the steps in the email to create your account

Setting Up Your Personal Portal Access

SELF-ENROLL ONLINE

  • Click this link: https://schealth.iqhealth.com/self-enroll/
  • Enter your name and date of birth exactly as it appears in our records
  • Verify your identity using: email address on file or social security number
  • Create your username, password, and security question
  • Note: self-enrollment will only work if your information matches exactly what we have on file.

GET AN INVITATION BY PHONE OR IN PERSON

  • Call Registration at 970-474-3323 or visit us in person
  • Tell us you want Patient Portal access
  • Provide your email address and answer a security question
  • Receive an email invitation (expires in 90 days)
  • Click “Accept Invitation” and verify your identity
  • Create your username, password, and security question

You're all set! Once complete, you can login from any device.

Setting Up Proxy Access for a Loved One

Parents, guardians, and authorized caregivers can access a loved one's health information.

  • Ask registration for "proxy access" if you're a parent, guardian, or caregiver
  • Complete the same setup process as personal access
  • If you already have an account, you can toggle between your records and your loved one's records
  • If you're a new user, you'll create an account to access only your loved one's information
