What to Expect

During your visit, your SCHC care team will:

  • Review your medical and family history
  • Assess risk factors for chronic conditions
  • Measure basic vitals (height, weight, blood pressure)
  • Health Risk Assessment questionnaire
  • Lifestyle counseling (nutrition, activity, tobacco use)
  • Update your list of medications and providers
  • Screen for memory concerns, depression, and fall risk
  • Create or update your own Personalized Prevention Plan
  • Preventive screening schedule (cancer, diabetes, etc.)
  • Immunization recommendations

Coverage & Eligibility

Medicare Part B covers your Annual Wellness Visit at 100% when provided by a participating provider — no copay and no deductible.

If additional services (like lab work or treatment of new concerns) are performed during your visit, those may be billed separately. We’ll always discuss this with you beforehand.

This visit does not include:

  • A full head-to-toe physical exam
  • Diagnosis or treatment of new medical problems
