Request Medical Records

You may request a copy of your medical records from Sedgwick County Health Center.

Patient Portal Access

Many records are also available in the patient portal, where you can securely view, download, and print available health information. If you need additional records, you may also send a message through the patient portal to request them.

How to Request Records

If you need records that are not available through the patient portal, or if you do not want to use the portal, follow these steps.

Download the Authorization Form

Download and Complete our Authorization to Release Health Information form. This is required for all formal records requests.

Complete the Form

Fill out the form completely to help us process your request as quickly as possible. Be sure to include:

  • Patient full name
  • Date of birth
  • Contact information
  • The records being requested
  • Date range of records, if applicable
  • Where the records should be sent
  • Signature of the patient or legal representative

For your privacy and protection, medical records requests require a signed authorization before records can be released.

If records are requested by email, Sedgwick County Health Center will use reasonable safeguards for transmission. Please note that we cannot control the security of a personal email account after delivery.
