Sedgwick County Health Center participates in the Hospital Transformation Program to strengthen care for Medicaid patients and improve outcomes for our broader community. Our focus is on better transitions of care, stronger communication, and support for patient needs beyond the hospital stay.

What HTP Means at SCHC

At Sedgwick County Health Center, HTP supports practical improvements in how we care for patients before discharge, during care transitions, and after they leave the hospital. This includes identifying social needs, supporting behavioral health and substance use care, scheduling follow-up services, and sharing important care information with primary care providers.
