Position Summary
 The Licensed Practical Nurse (LPN) at Sedgwick County Memorial Nursing Home provides compassionate, resident-focused nursing care in our 32-bed intermediate care facility. This role is responsible for administering medications and treatments, supporting daily living needs, monitoring changes in resident condition, and ensuring accurate documentation in alignment with each resident’s care plan. Working collaboratively with CNAs, RNs, providers, and families, the LPN helps create a safe, dignified, and supportive environment where residents can achieve their highest quality of life. The ideal candidate is a dependable caregiver who demonstrates strong clinical judgment, clear communication, and a commitment to teamwork, respect, and regulatory excellence in a close-knit rural care setting.

- Observes, monitors, and accurately documents resident symptoms, behaviors, reactions, and changes in condition, promptly reporting concerns to the Charge Nurse, Director of Nursing, or provider.Â
- Administers medications and treatments and assists providers with procedures in accordance with clinical orders, scope of practice, and facility policy.Â
- Completes comprehensive nursing admissions, ensuring all required forms, assessments, and documentation are accurate, timely, and consistent with regulatory and facility standards.Â
- Utilizes appropriate requisitions, forms, and clinical records to support resident care, maintaining familiarity with documentation tools and workflows used across nursing units as applicable.Â
- Actively participates in resident care planning by contributing observations, suggestions, and interdisciplinary insights to support individualized, person-centered plans of care.Â
- Adheres to established lines of communication and escalation pathways to support effective problem-solving, teamwork, and resident safety.Â
- Maintains open, respectful, and professional communication with residents, families, physicians, leadership, CNAs, and all members of the healthcare team to promote a positive care environment.Â

Qualifications and Skills
- Strong commitment to resident dignity, safety, and person-centered care principlesÂ
- Proficient in accurate medication administration, treatment performance, documentation, and follow-throughÂ
- Skilled in monitoring residents for changes in condition, identifying early warning signs, and notifying appropriate personnelÂ
- Strong computer and charting skills, including EMR use for documentation and communicationÂ
- Effective communication and interpersonal skills for engaging with residents, families, CNAs, providers, and interdisciplinary staffÂ
- Skilled in time management, prioritization, delegation, and working efficiently during varying workloadsÂ

- Ability to maintain professionalism, compassion, and patience when supporting residents with cognitive, behavioral, or functional challengesÂ
Experience Preferred
- Working knowledge of long-term care nursing practices, including medication administration, treatments, restorative care, and resident observationÂ
- Understanding of resident rights, abuse prevention, infection control, pressure injury prevention, fall prevention, and other major clinical risk areas in long-term careÂ
- Knowledge of regulatory requirements including CMS long-term care guidelines, HIPAA, and facility policies and proceduresÂ
- Familiarity with care planning, documentation standards, and electronic medical record (EMR) workflowsÂ
- Understanding of age-related changes and chronic conditions common among nursing home residentsÂ
- Awareness of emergency procedures, basic life support principles, and clinical escalation pathwaysÂ
Education and Certification Requirements
- Graduate of an accredited School of Nursing and current Licensed Practical Nurse (LPN) license in the State of Colorado (or compact state eligibility)Â
- Current BLS certification requiredÂ
- Prior experience in long-term care, skilled nursing, or geriatric care preferredÂ
- Ability to assess, plan, implement, and evaluate resident care in collaboration with interdisciplinary team membersÂ
- Strong commitment to resident dignity, safety, and person-centered care principlesÂ
Physical Requirements
 Frequent: sitting, standing, walking, & reaching.Â
Occasional:Â bending, squatting, lifting, carrying, pushing, & pulling.Â
Visual and hearing acuity required. Work is inside, with good ventilation and comfortable temperature.Â

Possible exposure to: toxic/caustic chemicals or detergents, moving mechanical parts, potential electric shock, radiant energy, communicable diseases, blood borne pathogens.Â

- Occasional (up to 0.5 hour per shift)Â
- Typical weight range:Â
- Up to 25 lbs: Common (e.g. supply boxes).Â
- 26–50 lbs: frequent, with assistance or proper equipment (e.g., helping transfer immobile patients or move equipment parts).Â
- Assistance required for anything over 50 lbs (e.g., repositioning bariatric patients or moving heavy equipment).Â
CarryingÂ
- Occasional (up to 0.5 hour per shift)Â
- Typical weight range:Â
- Up to 25 lbs: Standard (e.g., laptop, medications, wound supplies, etc.).Â
- Rarely exceeds 30 lbs. Use of carts encouraged for heavier items.Â
Pushing/PullingÂ
- Occasional to moderate (0.5–1.0 hour per shift)Â
- Typical exertion equivalent:Â
- Up to 50 lbs of force when moving portable machines, stretchers, or wheelchairs.Â
- Heavier loads (50–100 lbs) may occur but should be assisted with powered devices, staff help, or mechanical aids.Â

All positions of Sedgwick County Health Center are part of an interdisciplinary team, and as such, participate in the care and service delivery process through effective interaction with other team members. This position directly reports to the Director of Nursing.Â
